Well that's what I found on google, though it would look different if you translated it word by word into modern English you can still see what's the prayer about.
Word to word translation would look like this.
Healthy Mary
Full of Grace
Lord with you
Blessed you among "niewiastami" (Niewiasta in old Polish refers to unmarried women)
And Blessed your life's fruit, Jesus
Holy Mary, God's Mother
Pray for us sinners now and at the hour of our death.
Amen
I am not sure about the "Healthy" part though, I think "zdrowas" refers to "zdrowie" which means "health". I hope it helps ;)
